Before we begin the 2017-2018 season, we need to be sure everyone is up to date on their required paperwork.

- PIAA Athletic Physical with NHSD Waiver (dated after June 1, 2017)
- Social Media Permission Form
USA Hockey
- IMR Waiver - player/coach
- Birth Certificate for new NHHC members
PIHL
- Concussion Screening ...all players must be screened to participate and provide proof... age 13+ & tested every other year, under 13 every year, does not need to be retested...all others need tested & all new players. Please send proof that testing was completed.
North Hills Hockey Club (These forms were completed online during registration. The policies are available on our website. Please be sure to familiarize yourself and your player with them as they will be adhered to.)

Please be sure you have completed and turned in everything by 8/15/17. You will not be able to participate until all paperwork is completed.
