PIAA Sports Physicals - Must be completed and dated by a physician after June 1st.  Submit it directly to school via instructions sent from NHSD email on June 1.

E-communication form - Complete and return to northhillsregistrar@gmail.com

Concussion baseline testing - all players 12 and under every year.  Players age 13 and up every other year.  Send confirmation of completed testing to northhillsregistrar@gmail.com
